Patients are “constantly being inconvenienced, endangered,” or denied opportunities to improve their long term health because of a lack of joined-up care in the NHS, a report by the House of Lords has warned.1
The Lords’ committee on integration of primary and community care interviewed academic experts, government officials, former ministers, and representatives from healthcare, charities, and patient groups as part of a review into how primary and community services could be integrated to provide holistic care.
